Setting up your first appointment with us is as easy as 1-2-3.

1. Give us a call at 619.281.7800 to schedule your first appointment. If you will be using insurance make sure you have your insurance card available so that we can check your benefits before you come in.
2. Click the ‘New Patient Intake Forms’ link below to open up a PDF version of paperwork you will need for your first appointment. Filling it out before you come in saves you time and allows you to answer the questions more carefully.
3. Bring your completed paperwork, your insurance card, a photo ID, and comfortable loose fitting clothing for the exam. .
